FHN Offering $25 Sports Physicals for Area Schools

FHN healthcare providers are offering safe, hassle-free sports physicals at an affordable price for all area athletes, and all proceeds will be donated back to the schools affiliated with their local FHN clinics.

Physicals are $25, and meet eligibility requirements for all sports and are good through the 2025-26 school year.

Appointments are required unless otherwise noted; call the number listed with each event for more information or to make an appointment.

The following forms must be completed and signed by a parent or guardian before arriving for the physical:

• IHSA Pre-participation Examination Form
• Forreston Use ONLY
• Formulario de examen previo a la participación de IHSA en español
• FHN Consent Form
• IHSA Sports Medicine Acknowledgement and Consent Form

These exams meet Illinois eligibility requirements for all sports and are good through the 2025-26 school year. These exams also meet Wisconsin eligibility requirements.

Please Note: Area sports clinics will be added as soon as they are scheduled. If your school is not listed, please contact your local FHN Family Healthcare Center.

Requirements

• $25.00 upon registration via cash or check only. As in previous years,
  FHN donates this fee back to the school.
• All forms (FHN consent and IHSA exam) filled out and signed prior to coming to the sports physical.
• Only two adult support persons (one should be a parent/guardian) and athlete permitted in the building; no siblings or individuals not receiving a physical

Additional Information

• Approximate exam time is 15 - 20 minutes.
• Students will remain in one room for the exam.
• Exam areas will be cleaned after each student.
• Physical forms for all students will be turned over to the school at the end of each evening. Athletes can take a picture of the completed form with their phone for their records.
