BEFORE SERVICES

Is my insurance in-network at FHN?
Please utilize the Find a Provider feature on your insurance plans’ website and/or contact the Member Services Phone number on the back of your insurance card.

What if I am uninsured?
FHN has Financial Counselors who can assist with obtaining health insurance. For information on how to apply for health insurance under the Patient Protection and Affordable Care Act (ACA), sometimes referred to as Obamacare, please visit www.healthcare.gov.

How do I know what my services will cost?
To inquire about an estimate for the cost of services, please contact a Patient Financial Advocate at 815-599-8054.


DURING SERVICES

What financial obligations do I have during my appointment?
Your insurance may require that a copayment be collected at the time of service.

What is a copayment?
A copayment is a fixed amount you pay for a covered healthcare service that is due at the time services are rendered.

Here are some common medical services that require a copayment due at the time of service:

    • Primary Care Physician visits
    • Specialist visits
    • Emergency Room visits
    • Laboratory or other outpatient procedures

To understand your health insurance benefits, you should contact your insurance provider. The Member Services phone number is generally listed on the back of your insurance card.

Uninsured Patients
Uninsured patients will be required to pay a predetermined initial payment at the time of service.


AFTER SERVICES

How can I pay my bill?
You can pay your bill by utilizing the MyEasyMatch code on your billing statement, contacting Customer Service at 815-599-7950, or by meeting with a Financial Counselor at the Hospital Cashier’s Office or Burchard Hills.

What payment options do I have?
FHN offers Prompt Payment Discounts and interest-free payment plans.
